Hired Power’s Care Pathways Program provides community-based clinical support and care coordination for clients and families navigating mental health and substance use challenges. This is an ideal role for MFT graduate students seeking hours toward educational requirements, as well as AMFT/LMFT candidates.
WhatYou’ll Do
- Conduct client/family intakes and biopsychosocial assessments in home/community settings
- Coordinate care plans, referrals, and appropriate levels of care/placements
- Support treatment engagement and follow-through post-placement
- Provide structured family coaching and education
- Collaborate with multidisciplinary providers and internal teams
- MFT students/trainees: Supervision and case consultation aligned with training requirements
- AMFT: Weekly BBS-compliant supervision provided
- LMFT: Clinical consultation as assigned
- Interest/experience in community-based mental health and/or substance use work
- Strong communication, boundaries, and documentation skills
- Valid CA driver’s license, reliable transportation, ability to travel locally
- Family systems experience, Motivational Interviewing, trauma-informed approach
