POSITION SUMMARY – RN Case Manager

POSITION DUTIES

• Discharge Planning
• Care Coordination
• Continuum of Care
• Patient Focus and Care Plans
• Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS

• FL/compact RN license; BLS
• 1+ year case management experience in an acute care setting
• Discharge planning for complex patient needs
• Knowledge of multidisciplinary rounds and what is required to meet LTACH, SNF, HHC level of care to give appropriate referrals
• Interqual knowledge

Preferred Qualifications – CCM

• CCM preferred

LENGTH OF ASSIGNMENT – 8 weeks

SHIFT / HOURS PER WEEK – 8-4:30 M-F with every other weekend

SYSTEMS – Meditech / Midas / NAVI

START DATE – 2/16/2026
