Position: Outpatient Program Director (PT or PTA)

Position Summary

We are seeking a licensed and experienced Outpatient Program Director to lead and grow therapy operations at one of our senior living communities. This role blends clinical oversight, program development, and administrative leadership, with a strong emphasis on developing and sustaining an outpatient caseload through referrals and relationship-based outreach.

The ideal candidate is a licensed therapist (either a Physical Therapist or Physical Therapy Assistant) who thrives in a growth-focused environment. Rather than inheriting a full schedule, you will play an active role in developing referral pipelines, increasing program visibility, and establishing a strong outpatient presence within the ALF and surrounding community.

You will enjoy this role if you are energized by building something meaningful from the ground up. Success in this position comes from being visible, engaged, and proactive—earning referrals through trust, collaboration, and consistent clinical excellence. This is a hands-on leadership role where clinical care, outreach, and operational ownership work together to drive program growth.

Key Responsibilities
  • Provide leadership and day-to-day management of therapy staff and outpatient clinic operations
  • Actively build and grow the outpatient caseload through referral development, relationship building, and on-site presence
  • Lead resident events, educational sessions, and in-services to promote therapy services and drive referrals
  • Maintain strong communication with regional leadership, referral sources, facility partners, and internal teams
  • Oversee therapist schedules and staffing, aligning hours with caseload volume and program demand
  • Ensure accurate intake, screening, insurance verification, and referral processing
  • Support compliance with documentation standards, payer requirements, and regulatory guidelines
  • Serve as a liaison for residents, families, therapists, and facility staff
  • Provide direct patient care within the scope of licensure as needed to support caseload growth
  • Maintain supply inventory and organized clinical space
  • Adhere to HIPAA standards and company confidentiality policies
  • Participate in facility meetings, performance reporting, and leadership initiatives
  • Promote a culture of accountability, collaboration, growth, and clinical excellence
Qualifications
  • Valid, unencumbered professional license as a PT, OT, PTA, or COTA
  • Ability to be credentialed with all insurance payers
  • CPR/BLS certification required
  • Program Director or leadership experience in a senior living or outpatient setting strongly preferred
  • Comfort with referral development, relationship-building, and program growth responsibilities
  • Strong interpersonal, organizational, and leadership skills
  • Ability to balance clinical care, staff leadership, and business development
