• Assists with palliative care consultations and interventions for early initiation of chronic disease management related to life-limiting conditions.
• Advocates for patients and families by initiating referrals to home care or hospice services for eligible patients.
• Provides education related to palliative care to patients, families, community, and healthcare professional groups.
• Performs initial palliative evaluation with patient and family, including health history and psychosocial determinants.
• Participates in quality monitoring, regulatory compliance, and risk management performance improvement activities to ensure continuous improvement.
