LPN Home Health: FT/PT/PRN: 7am-3pm (ability to work Sunday is a plus)

Job Summary

The licensed practical nurse provides services for clients consistent with the facilities philosophy, policies and standards of care under the direction of the registered nurse.

Essential Functions and Responsibilities
  • Observe, evaluate and provide nursing care in accordance with physician orders and established plan of care to meet the physical, social and psychological needs of the residents
  • Give direct nursing care, utilizing skill and judgment, including therapeutic measures prescribed and delegated by the physician within scope of practice
  • Interprets to the resident/client or family/caregiver policies, procedures, treatments or therapeutic measures within the scope of knowledge and practice
  • Administers medications and treatments prescribed by physician in accordance with level of practice and agency policy and procedure
  • Prepares and updates list of medications for clients receiving medication administration or a medication pour
  • Reorder medications for clients through the pharmacy or through mail order
  • Check medication administration records daily to see that all shifts are administering medications and performing treatments as ordered by the physician
  • Prepare and check all medication administration records for the next month, make the appropriate changes in the computer, initial and date each record after it has been reviewed for accuracy
  • Follow-up on lab results, call MD’s and coordinate new orders
  • Report to the 3P-11P nurse any pertinent information related to medication administration and med pour clients
  • Complete a visit note for each visit
  • Document and report abnormal signs/symptoms or changes in condition to the registered nurse and/or the physician
  • Maintain a log of activities during the day such as calls to physicians, families and pharmacies
  • Make recommendations to the registered nurse for additional or diminishing visits according to observed needs
  • Communicates with the office throughout the day for new orders, labs and other communication
  • Works and cooperates with other departments in the delivery of services and care to residents/clients
  • Maintain awareness of Medicare reimbursement criteria for documentation of skilled nursing visits
  • Clinical notes, time tickets and medical supply vouchers are submitted on time per agency policy
  • Routinely provide assistance to residents which requires lifting, pushing and balancing the equivalent of 50 pounds or greater
  • Follow agency problem solving mechanism
  • Participate in case conferences, in-services and staff meetings
  • Participate in evaluation of services and quality assessment activities
  • Cross train and cover other areas such as the Clinic and Medicare Home Health as needed
Other Functions and Responsibilities
  • Other duties as requested or assigned by registered nurse or Director of Nursing.
Working Conditions
  • Travel required during all seasons to assigned client/patient homes and work in various home conditions
  • May be exposed to noises or sound levels that are distracting. Medical equipment can sometimes be loud
  • Work very near others. Routinely comes in physical contact with patients throughout your shift
  • Often exposed to contaminants
  • Often exposed to diseases or infections from contact with clients
  • Sometimes wear safety gear, such as gloves, face masks, and back support
  • May have to get into awkward positions to reach cramped work spaces
Educational, Physical, and Other Requirements
  • Must pass Level 2 background screening through the Florida Clearinghouse (Learn more: )
  • Employee must be able to perform the essential functions of the job with or without reasonable accommodations. Physical requirements may include frequent use of hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; talk or hear. The employee may be required to lift, stand; walk; sit; climb or balance; stoop; kneel; crouch or crawl; and smell.

    Vision abilities may require close vision, distant vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
