Reports to: Family Services MiFT Supervisor – Traverse City

Wellspring is a statewide nonprofit dedicated to serving seniors, families, and children throughout Michigan. We are currently seeking a Family Preservation Social Worker to join our MiFT Program in Traverse City, providing direct support to families across Michigan’s Northwest Region.

• Adhere to the MiFT model of service using a strengths-based, non-deficit approach.
• Provide families with concrete services including housing support, transportation, home cleaning, maintenance, and repair.
• Deliver skills training in parenting, communication, self-advocacy, and self-sufficiency.
• Provide short-term counseling and crisis intervention.
• Coordinate with external providers to arrange counseling, medical care, educational support, housing resources, and utility assistance.
• Prepare timely documentation including service plans, progress reports, termination summaries, goal sheets, and weekly reports.
• Present cases for termination conferences and recommend follow-up services.
• Maintain 24-hour availability for clients and work a flexible schedule based on family needs.
• Conduct face-to-face contact with families according to contract requirements and family needs.
• Maintain weekly communication with referring workers and complete follow-up evaluations at 3, 6, and 12 months post-service.
• Transport clients using a personal vehicle as needed.

• Bachelor’s degree in social work or a related human services field (required).
• Valid driver’s license, vehicle registration, and auto insurance (required).
• Experience working with children and families (preferred).
• Reliable, compassionate, and committed to providing high-quality services.
• Strong organizational, communication, and problem-solving skills.

• Employment Type: Full-Time
• Schedule: Flexible schedule with 24-hour availability as needed
• Work Environment: Hybrid – home-based with daily travel throughout the service area
• In-Office Requirements: Weekly team meetings, trainings, and clinical supervision
• Salary: $44,500 / year
