Job Title:Adults Community Social Worker – Working Age Adults Team
Location:Milton Keynes
Rate:Up to £35 per hour

An exciting job role has recently become available at Milton Keynes. They are looking for a new member for their Working Age Adults Team.

Responsibilities:
Assess individuals in the community for care packages or support services. Collaborate with other professionals to create care plans. Provide support to adults with various needs, including physical disabilities, mental health issues, and learning disabilities. Engage in carers conversations. Manage safeguarding enquiries and ensure client safety. Participate in duty and duty back-up rota as required.

Qualifications and

Experience:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
