Location: Hybrid / Remote – Gaylord
Service Area: Otsego, Antrim, Charlevoix, Emmet, Cheboygan, Crawford and Kalkaska counties

Wellspring is a statewide nonprofit dedicated to serving seniors, families and children throughout Michigan. We are currently seeking a Family Preservation Social Worker to join our Family Reunification Program in Gaylord. In this role, you will provide direct support to families throughout Northern Michigan, working in homes and communities to promote family stability and reunification.
Responsibilities- Adhere to the Wellspring Family Reunification Program model using a strength-based, non-deficit approach
- Provide concrete services including assistance with housing, transportation, home cleaning, maintenance and repair
- Deliver skills training in parenting, communication, self-advocacy and self-sufficiency
- Provide short-term counseling and crisis intervention as needed
- Coordinate services with community providers including counseling, medical care, education, housing and utilities
- Complete timely documentation including service plans, progress notes, termination summaries, goal sheets and weekly reports
- Present cases at termination conferences and recommend appropriate follow-up services
- Maintain 24-hour availability and a flexible schedule based on family needs
- Conduct face-to-face contact with families based on contract requirements and family need
- Maintain weekly communication with referring workers and complete 3-, 6- and 12-month follow-up evaluations
- Transport clients in your personal vehicle as required
- Bachelor’s degree in social work or a related human services field required
- Valid driver’s license, vehicle registration and auto insurance required
- Experience working with children and families preferred
- Reliable, compassionate and committed to high-quality service delivery
- Strong organizational, communication and problem-solving skills
- Employment Type: Full-time
- Schedule: Flexible with 24-hour availability as needed
- Hybrid-Remote: Home-based with travel throughout the service area
- In-Office Requirements: Weekly team meetings, trainings and clinical supervision
- Pay Rate: $42,000 annually, based on experience
