About the Post

We have an exciting opportunity for an Assistant Social Care Practitioner to join our Hospital Social Work Team in Barnsley. We are looking to appoint a highly motivated, enthusiastic, and committed individual to join our established team of Social Workers and Assistant Social Care Practitioners.

The role will entail working with individuals, families, carers, and communities to enable them to make informed decisions, to promote independence, whilst working in a strengths‑based approach. Undertaking collaborative working with Health colleagues through the different Discharge To Assess Pathways is also a key part of the role. You will be responsible for completion of Care Act Assessments and construct value for money care packages, and commission and arrange appropriate services.

Knowledge of the Discharge to Assess Process is advisable.

As an Assistant Social Care Practitioner in our Hospital Social Work Team, you will be a relatable practitioner responding to situations with strengths‑based practice at the heart of your work. You will develop creative and imaginative solutions to help people achieve positive outcomes leaving hospital / the difference Discharge to Assess Pathways which enable people to maintain their independence and live a happier healthier life.

Along the way, you’ll make your own progress through training opportunities and programmes.

What you’ll need

To join us, you’ll need a relevant Level 3 Qualification in Health and Social Care.
