We are currently recruiting an experienced Social Worker to join the Adults Locality Team within Wiltshire Council. This is an exciting opportunity to join a well‑structured, supportive and fast‑paced team that is expanding its workforce. • Work closely with professionals across health, voluntary and statutory sectors as part of a multi‑disciplinary team
  , ensuring effective communication and shared best practice.
• Review, monitor and evaluate care plans and the quality of service provision, responding proactively to changing needs.
• Develop, implement and assess reablement programmes within care packages, promoting independence and positive outcomes.
• Manage your own caseload or specialist projects
  , ensuring appropriate interventions and timely outcomes.
• Complete risk assessments
  , safe methods of work practice guides, and raise awareness of a wide range of care‑related issues and their implications.
• Undertake visits across the community, including home visits
  , as required.

• Qualified Social Worker with relevant experience in Adult Social Care
• Full UK driving licence and access to a vehicle (or ability to travel via other suitable means)
• Ability to work flexibly, with the role being predominantly office and community‑based
  , alongside some home working
