Salary: £39,129 - £46,023 (pro-rata for part-time)

Hours:

26 per week, 52 weeks
For social work qualification – MHO and Practice Educator qualifications attract an additional salary supplement.

An exciting opportunity has arisen to work within HBCCC/ICF (Hospital Based Complex Clinical Care/Interim Care Facility) Hospital Social Work Team as a Social Worker. We are looking for highly motivated and enthusiastic Social Workers to join our fast-paced and dynamic service which provides a responsive and person-centred service in supporting timely and safe discharges for adults from non-acute hospitals facilities.

The Edinburgh Health & Social Care Partnership is on a journey to transform the way in which Social Work delivers support and services to ensure that we support adults with multiple and complex needs, and increasing levels of frailty, to live as independently as possible.

We work effectively with our health colleagues and a broad range of partners to ensure that the most vulnerable within our city get the right service in the right place at the right time. We have a key priority to shift the balance of care and promote effective early intervention and preventative supports.

This post is regulated work with children and/or protected adults under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidate will be required to join the PVG Scheme or undergo a PVG Scheme update check. Where an individual has spent a continuous period of 3 months or more out with the UK in the last 5 years, an Overseas Criminal Record Check will be required.

You will be required to provide this check. An unconditional offer of employment and commencement in the post will be subject to the outcome of both these pre‑employment checks being deemed satisfactory.

Establishing, monitoring and reviewing care plans.

Multi disciplinary working

Working with adults

Experience of work with dementia, carers. Experience of working in a community team. Experience of working with older people.

Experience of adult protection work

Knowledge, skills and understanding

Knowledge of current legislation and policy. Good communication skills.

Ability to prioritise workload, work under pressure and meet deadlines.

Skills SWIFT /AIS or similar caseload recording and management systems.

Understanding management of budgets and development of individual support plans

Qualifications and training

Diploma in Social work. Registration with the Scottish Social Services Council.

Adult Protection Training

Dementia specific training. Practice Teaching/link supervisor qualification.

Mental Health additional training

Job specific requirements

Recognises and works within professional boundaries.
