Human Services Case Manager 2 (Family Housing Assistance Program)
Location: 647 1st Avenue North, St. Petersburg, FL 33701
Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.; occasional evenings/weekends as needed. Eligible for partial remote work after satisfactory performance, in accordance with County policy.

The Family Housing Assistance Program (FHAP) is a Housing First, Rapid Re‑Housing program that plays a critical role in supporting families experiencing homelessness to quickly access and sustain safe, stable housing.
Grounded in trauma‑informed, culturally responsive, and strengths‑based practices, the Case Manager 2 partners with families to identify goals, build on existing capacities, and remove barriers to housing stability and economic mobility. Through individualized case management, time‑limited financial assistance, coordinated service connections, and workforce development support, the Case Manager 2 helps families transition from homelessness to stable housing while promoting self‑sufficiency and measurable outcomes in housing stability and income growth.

What Would You Do?Case Management: Provide case management services for clients in your caseload (average caseload size is 15–20). Utilize your skills to assess, plan, coordinate, monitor and advocate for the needs of clients. Be a guiding force in connecting families with vital resources and support. Conduct in‑home, shelter, and community‑based meetings with families.
Assessment and Planning: Conduct comprehensive assessments to identify client needs, strengths, and goals. Develop individualized client service plans in collaboration with families and interdisciplinary teams. Maintain consistent communication to support engagement, accountability, and proactive problem‑solving.
Coordination and
Collaboration:
Build supportive, professional relationships with families. Collaborate with internal teams and community partners to coordinate services, facilitate financial assistance payments, and ensure a seamless support network. Assist families in locating and securing appropriate housing, including landlord engagement, housing applications, lease review, and negotiation as needed.Monitoring and Evaluation: Regularly review and update client progress, adjusting plans as needed. Monitor service delivery, ensuring it aligns with established goals and objectives. Adhere to confidentiality and privacy standards.
Client Empowerment: Empower clients to navigate challenges and build resilience. Foster a supportive environment that encourages growth and positive outcomes.
Educate: Provide clients with information, self‑management strategies, and empowerment techniques to enhance their ability to navigate and work through challenges independently. This includes support in budgeting and financial literacy skills, basic life and home care skills (with training as needed), job search, resume development, and interview preparation. Also, build connections with potential employers, educational institutions, and training programs.
Advocacy: Be the voice for those who need it most. Advocate for clients' rights and access to essential services within the community.
