We are looking for a passionate, supportive and hardworking social worker who is interested in working within an organisation that is committed to providing an exceptional and integral service to the communities of Lincolnshire. You will make a difference and develop your skills and experience.
This is an exciting role, available due to career progression, working closely with community health services to support vulnerable service users.
You will be part of the East Community Hospital and Transfer of Care team with a specific remit to support those Lincolnshire residents receiving services from Lincolnshire Community Health Services bedded rehabilitation, the Home Based Reablement Service and homecare Prime Providers. You would be working with Multi Disciplinary Teams and system partners.
Main duties of the jobYou will undertake assessments in line with the Care Act, Mental Capacity Act and you will support colleagues within Adult Social Care such as the safeguarding team to ensure the wellbeing of everyone you work with. You will also provide peer support across the team and supervision to non-qualified staff in the team.
You will show sound knowledge of relevant legislation underpinning statutory Adult Care Practice, Social Work theory and be able to demonstrate values that promote wellbeing, challenge oppression and discriminations and empower people to live well.
You must hold a professional Social Work qualification e.g.DIPSW, CQSW, CETSW, CSS or BA Social Work and ensure that registration with Social Work England is maintained and renewed appropriately.
