What you will do:

– Build strong relationships with children, young people, and their families to understand their unique needs.
– Assess the needs of children, young people, and their families to develop effective and personalised support plans.
– Collaborate with partner agencies and co-develop intervention plans that address the needs and goals of families.
– Provide consistent and impactful support and interventions within the community setting.
– Work closely with family support workers and partner agencies to deliver comprehensive and holistic interventions.
