Overview

Brief Description Position Summary:
The Safe Homes Service Coordinator is responsible for assessing, accepting, and providing comprehensive support to participants in our community-based rapid re-housing program. This role involves welcoming and orienting new residents, crisis intervention, and safety planning, and developing individualized intervention strategies. Additionally, the Coordinator fosters relationships with landlords and facilitates communication among program partners. Offsite and home visits are integral to this role.

Given that all participants are victims of Intimate Partner Violence with complex traumatic experiences, the Coordinator must possess a deep understanding of trauma and be adept at engaging with traumatized individuals. The position operates Monday through Friday, from 9 AM to 5 PM, and is based at the Argonne Drive campus in Baltimore City. This role is on-site with the potential of being hybrid, with three days in the office and two days working from home or in the community.

Travel within Baltimore City, Baltimore County, Prince George’s County, and Montgomery County may be required up to 30% of the time.

Essential Duties And Responsibilities
  • Manages a caseload of approximately 20-25 participants enrolled in the Safe Homes rapid re-housing project, providing culturally sensitive coordination of services.
  • Conducts thorough assessments, develop comprehensive service plans, and coordinates with internal and external service providers. Conducts thorough assessments, develop comprehensive service plans, and coordinates with internal and external service providers.
  • Provides three months of follow-up support after the rental assistance period ends.
  • Assesses safety needs and makes recommendations for improvement.
  • Collaborates with community partners to ensure comprehensive service delivery.
  • Maintains accurate case files and assists in compiling necessary data for grant reports.
  • Guides and supports participants to increase self-sufficiency skills, safety, education on domestic violence, and problem solving.
  • Educates immigrant participants on American processes and systems including teaching self-advocacy skills and advocating on behalf of participants when necessary to achieve participant goals.
  • Assists program management in compiling necessary statistical data and narrative for grant reports and applications.
  • Facilitates participants' access to agency and community services, organizing team meetings as needed.
  • Visits participants in home environments that have been determined to be safe, including temporary hotel placement.
  • Participates in team meetings and makes recommendations concerning individual participant’s length of stay in the program and eligibility for financial assistance, including recommendations on distribution of stipends.
  • Provides comprehensive support to participants by assisting them in planning their program exit and facilitating connections with other resources, while also offering 3-6 months of post-funding service coordination to ensure a smooth transition after the rental assistance period concludes.
  • Provides one on one counseling for participants in crisis and to help participants resolve conflict.
  • Distributes material goods according to program procedures.
  • Provides relief and back-up coverage in emergencies.
  • Distributes material goods and financial stipends to program participants.
  • Creates service plans with participants. Reviews and adjusts plans as relevant.
  • Makes offsite visits, and visits to agency providers and landlords.
  • Recommends participants for admission to HRM programs, including Emergency Shelter or 3 day emergency hotel placements pending space.
  • Represents the agency with area service providers and landlords.
  • Arranges for interpreters for participants as needed.
  • Meets program quality and service targets, including caseload performance standards and data entry deadlines.
  • Contacts assigned clients within 24 hours and complete Intake assessments and service plans within 7 days of program acceptance.
  • Tracks all client contacts within 48 hours.
