This is a full-time hybrid role for a Hospital 2 Home Case Manager/Social Worker based in the Las Vegas Metropolitan Area, with flexibility for some work-from-home arrangements. The Case Manager/Social Worker will manage client cases to ensure they receive adequate services, support, and resources. Daily responsibilities include conducting needs assessments, developing care plans, connecting clients with local resources, coordinating services, and advocating for the well-being of older adults and their caregivers.
The role also involves monitoring progress and maintaining accurate documentation.
- Experience in case management and social work, with skills in care planning and client advocacy.
- Strong interpersonal, communication, and active listening abilities to build relationships with clients and community partners.
- Knowledge of local resources, aging services, and community programs available in Southern Nevada.
- Proficiency in maintaining accurate records, using case management software, and general computer literacy.
- Time management, organizational skills, and ability to handle multiple priorities effectively.
- Experience in working with vulnerable populations, older adults and/or individuals with disabilities is preferred.
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field is required;
Master’s degree is a plus. - Valid driver’s license and reliable transportation for community visits is required.
