Supervisory Social Worker; HBPC & Foster Care Programs

Overview

The incumbent is assigned to Home Based Primary Care (HBPC) and the Medical Foster Home Program (MFH). The Supervisory Social Worker will serve in combined clinical and supervisory roles for both HBPC and MFH, providing services in the community, Columbia VA Health System as well as the VA Outpatient Clinics (Florence, Sumter, Orangeburg, Greenville, Spartanburg, Anderson, and Rock Hill).

• The Supervisor Social Worker will provide high quality case management and mental health care and services through the oversight of HBPC Social Workers, HBPC MH Social Workers, and Medical Foster Home Social Workers who are assigned to Veteran patients enrolled in the HBPC and Medical Foster Home programs. This oversight includes ensuring supervises provide active case management and care coordination, clinical assessment and treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers/agencies as needed by the Veteran.
