** Summary
* * Under general direction of the Preventative Health Division, provide case coordination and case management services to adults who are 60 years of age or older who have been a victim of a crime, or of physical abuse, sexual abuse, financial exploitation or caregiver neglect. Case managers will be expected to attend Senior Justice Assessment Center (SJAC) meetings weekly, monthly, and quarterly as directed.
Working conditions consist of a hybrid of field work, an office environment, and remote work as necessary. Case managers will be conducting frequent home visits, facility visits, fieldwork and office visits with clients, and will need to be mobile throughout the County as necessary.
Case Managers provide support to victims in crisis, conducts assessments, educate on the legal process, make appropriate referrals, educate clients on victims’ rights and the dynamics of elder abuse. Case managers will participate, attend, and coordinate multi-disciplinary case review meetings weekly with key stakeholders. Case managers will be working in an office setting, performing fieldwork and office visits with clients, and be mobile throughout the County as necessary.
** Duties and Responsibilities
*** Completes face-to-face visits and psycho-social assessments for the SJAC
* Prepare and maintains comprehensive, accurate and timely case documentation entered into case management system
* Coordinates with core and auxiliary SJAC agencies, organizations, and vendors to provide appropriate services and information to clients
* Build and maintains strong working relationships with key partners such as law enforcement officers, attorneys, physicians, and other team members to ensure a collaborative investigation and victim restoration process are met.
* Provide ongoing case coordination and regularly present case information in a multi-disciplinary setting
* Develops casework/service plans and monitors progress by conducting follow-ups with clients based on client need
* Conducts assessments of client needs by providing crisis intervention, safety planning, and case management services for clients
* Conduct risk assessment, needs assessment, and safety plans with families. Be aware of local community resources and make appropriate referrals.
* Coordinates with other social service agencies, organizations, and vendors to provide appropriate services and information to clients
* Provides advocacy on behalf of the client by making appropriate referrals to other social service, education, health agencies, and navigating the criminal justice system as needed by client.
* Upon supervisory approval, plans, organizes, and participates in special needs projects including outreach efforts, public speaking, and special activities to clients and the Bexar County community
* May perform other related duties as required

* Minimum Qualifications:
*** Graduation from an accredited college or university with a Bachelor's Degree in Psychology, Counseling, Social Work, Criminal Justice, or Criminology with three (3) years of experience in social services-related work and or an equivalent combination of education and experience
