The Role We are currently recruiting an experienced Social Worker to join the Adults Locality Team within Swindon Borough Council. This is an exciting opportunity to join a well-structured, supportive and fast-paced team that is expanding its workforce.

Key Responsibilities
Work closely with professionals across health, voluntary and statutory sectors as part of a multi-disciplinary team, ensuring effective communication and shared best practice.
Review, monitor and evaluate care plans and the quality of service provision, responding proactively to changing needs.
Develop, implement and assess reablement programmes within care packages, promoting independence and positive outcomes.
Manage your own caseload or specialist projects, ensuring appropriate interventions and timely outcomes.
Complete risk assessments, safe methods of work practice guides, and raise awareness of a wide range of care-related issues and their implications.
Undertake visits across the community, including home visits, as required.
Requirements
Qualified Social Worker with relevant experience in Adult Social Care
Full UK driving licence and access to a vehicle (or ability to travel via other suitable means)
Ability to work flexibly, with the role being predominantly office and community-based, alongside some home working
