We are seeking an experienced and motivated Social Support Program Lead to plan, deliver and continuously improve high-quality social support programs for older people and younger people with disabilities in our community.
This is a rewarding leadership role where you will combine program coordination, staff support and client engagement to ensure inclusive, safe and responsive services aligned with Commonwealth Home Support Programme (CHSP) and Home and Community Care Program for Younger People (HACCPYP) funding requirements.
About the RoleReporting to the Coordinator Active Ageing, you will lead the delivery of Council s Social Support Program, ensuring eligible clients are supported to participate through effective onboarding, care planning and co-designed program activities. You will also provide leadership to support staff, manage program logistics and oversee financial and operational requirements.
Hours and Location- 3 days per week- ASAP - end of June 2026
- 2 days on program non-negotiable days Tuesdays and Wednesdays 8.30am - 4pm
- Program days are located at 54 Raleigh Road Maribyrnong
- 3 rd day at Raleigh Road or Footscray or Work from home (hours negotiable)
- Plan, coordinate and evaluate Social Support Programs using a co-design approach
- Manage referrals, care plans, client communication and system records
- Lead, support and guide staff, casuals and volunteers to deliver high-quality programs
- Oversee program logistics including transport, bookings, site visits and risk assessments
- Manage program budgets, purchasing and reporting requirements
- Administer and liaise with providers regarding the CRES system
- Prepare reports, briefings and correspondence related to Social Support programs
- Ensure compliance with relevant legislation, funding guidelines and Council policies
You are a capable and compassionate professional with experience in the care and community services sector. You bring strong organisational skills, confidence in leading people, and a commitment to inclusive, person-centred service delivery.
Key Selection Criteria- Minimum Certificate III in Individual Support (Aged Care) or equivalent
- Several years experience working in the care services industry
- Demonstrated experience coordinating programs for people with physical and/or cognitive support needs
- Proven ability to lead and support staff to deliver high-quality services
- Strong skills in planning, costing and resourcing programs
- Excellent communication, customer service and stakeholder engagement skills
- Ability to work autonomously and collaboratively in a multidisciplinary team
- Current First Aid and CPR Certificate
- Current Victorian Driver Licence
- Current Working with Children Check
- Satisfactory Police Check
