• Assist families with basic needs and resources to promote self-sufficiency
• Develop service plans with measurable goals
• Provide in-home services when appropriate
• Visit each client at least once per week
• Facilitate networking and linkages with other community services
• Maintain accurate case notes and records
• Complete referral forms, social histories, and closure reports within designated time frames
• Participate in team meetings and staff reviews to evaluate client progress
• Follow up on referrals and coordinate necessary services

• Education:
  
  2 years of college at an accredited institution (Associate s Degree or higher preferred)
• Experience:
  
  Knowledge of trauma-informed care and willingness to engage in ongoing training
• Requirements:
• Valid driver s license, reliable vehicle, and state-required car insurance
• Ability to pass State and Federal background checks
• Strong communication and organizational skills
• Ability to work independently and within a team
• Comfortable providing home-based services to families
