The Adult Social Care Transitions Team (14-25) provides social care assessment, case management, and safeguarding support across Staffordshire, working alongside young people living with disabilities and additional needs, including neurodiversity and mental health challenges. The team collaborates with SEND and education provisions to support transitions, involving Social Workers, Social Care Practitioners, children’s social workers, SEND workers, and health and education professionals.

The team's focus is to support young people in experiencing a successful transition into adult social care, helping to prevent abrupt changes for those they support.

This team is dedicated to ensuring the best possible transition for young people. The Senior Social Worker role is vital in supporting the workforce and ensuring strength-based practice. The role is based in Stafford town centre (Staffordshire Place
1), requiring attendance at the office at least twice a week, with the option for home working on other days.

Interviews are scheduled for the week starting 21 July 2025 and will be conducted in person.
