Band D: £32,597 - £37,280 per annum

Full Time – 37 hours per week (flexible working patterns available)

We are currently seeking a passionate and skilled Family Support Worker to join our Children with Disabilities EHCP Social Care Team. This is a rewarding opportunity to support children and young people with Education, Health and Care Plans (EHCPs), helping ensure their social care needs are understood, assessed and met through a restorative, strengths-based and evidence-informed approach.

The hours are flexible and can be discussed to meet the needs of both the successful applicant and the service.

About the Role
  • Working under the direction of a Team Leader, you will:
  • Support the social care contribution to EHCPs for children and young people with disabilities
  • Undertake direct work with children, young people and families to capture their views, wishes and feelings
  • Support social care assessments through visits, information gathering and report writing to support with short breaks including packages of care
  • Prepare written contributions for EHCPs and assist with RAP documentation
  • Provide practical and administrative support in arranging care packages
  • Signpost families to community resources, the Local Offer, and voluntary sector support
  • Work closely with schools, health professionals and partner agencies
  • Be alert to safeguarding concerns and escalate appropriately
  • Maintain high-quality, timely records in line with Directorate policies

This role offers the chance to make a tangible difference in the lives of children with disabilities and their families, while working within a supportive, reflective and values-led team.

About You

You will have:

  • A Level 3 Diploma for the Children and Young People’s Workforce or an equivalent qualification
  • Experience of working on a 1:1 basis with children, young people or families in a social care, education or voluntary setting
  • An understanding of the additional vulnerabilities and complex needs of children and young people
  • The ability to build positive, collaborative relationships with children, young people and families
  • Strong written and verbal communication skills
  • Emotional resilience and a commitment to ongoing learning and development
  • A full driving licence and access to a car (reasonable adjustments will be made for disabled applicants)

Experience of working with children with disabilities, contributing to assessments, or supporting families to bring about change would be advantageous.
