Position Summary
This position provides professional case management services to individuals and families participating in the Denver Metro Homeless Prevention Program, promoting housing stability and long-term self-sufficiency. The Case Manager conducts comprehensive client intake assessments, develops and implements individualized service plans, and provides ongoing counseling, crisis intervention, and supportive services. Requires collaboration with clients and community partners, continuous monitoring and adjustment of service plans, and creativity and innovation in program development and service delivery to effectively meet client needs.

DescriptionThis position is Full‑Time, grant‑dependent, 37.5 hours per week.

Hiring range: $24–$29 per hour.
Essential Functions- Assists with the CRS rent assistance phone system and client calls.
- Provides information and referrals to callers.
- Performs comprehensive psycho‑social assessments of adults, children, and families.
- Assists with client intake assessments and processes.
- Develops, implements, monitors, and modifies treatment/service plans with clients.
- Provides counseling, crisis intervention, and supportive services to assist clients in reaching treatment goals.
- Inputs client information into databases as required.
- Requests financial approval for services of the program based on assessment of need and availability of resources.
- Maintains accurate case records; documents client contacts and progress to ensure accountability.
- Allows new staff and interns to shadow and to sit in with client appointments.
- Helps develop and maintain department programs and services.
- Works with other staff to ensure proper completion of paperwork and adherence to grant criteria.
- Assists with reporting as needed.
- Collaborates with community prevention agencies.
- Participates in MHUW work groups, as well as any other funder‑required committees/work groups.
- Participates in staff trainings as required.
- Completes or provides input to reports documenting program activities.
- Adheres to all grant eligibility and documentation requirements.
- Uses creativity and innovation in program development and service delivery.
- Maintains confidentiality of client and Agency information.
- Assists with special department programs.
- Assists with intakes, services, and case management needs for Weinberg Food Pantry clients.
- Collaborates with community prevention agencies.
- Other duties as assigned.
Off‑Site & In‑Office Position:
Work primarily takes place in the community with occasional requirements to be at the home office at 3201 South Tamarac Dr., Denver 80231. Work hours are generally during regular business hours, with occasional flexibility required to support agency needs.
Essential physical requirements:
Frequent sitting and typing. Regularly required to use standard office equipment. Travel between sites, commute, use standard mobile equipment.
Physical effort/lifting:
May occasionally be required to lift up to 10 pounds.
Must be fully vaccinated for COVID‑19 (proof required), subject to legally required exemptions.

Qualifications- Education/Experience
- Preferred bachelor's degree in social work, related field, or equivalent work experience.
- Preferred three plus years’ experience as a Social Case Worker for a public or private agency.
- Knowledge and understanding of the Denver metro area resources is preferred.
- Bilingual in Spanish is strongly desired.
- Strong communication, organizational, and interpersonal skills.
- Ability to interact effectively as a team member and independently with Agency staff and with a diverse client base.
- Ability to multitask in a fast‑paced environment.
- Must be proficient in Microsoft Office Software.
$24.00 – $29.00 per hour.
