• Act as a liaison between the Justice System, Support Coordination team, and justice partners for coordinated re-entry services
• Maintain documentation and statistical reports related to members involved in the justice system
• Respond to inquiries and provide resources to family members and community members
• Present the Justice Reach-In Program to stakeholders and facilitate monthly Justice Committee meetings
  
  Ensure successful transition of individuals back into the community post-release

• Must possess a valid driver's license and maintain acceptable driving records
• Ability to pass background and reference checks, including fingerprint clearance
  
  Knowledge of the DDD service system and community resources
• Strong oral and written communication skills
• Experience working with stakeholders and families in a professional manner

• Experience in developmental disabilities, specifically in Support Coordination
• Familiarity with AHCCCS programs and the Justice System
