• Face‑to‑Face Assessments:
  Conduct in-person assessments and provide tailored rehabilitation support, primarily across Doncaster.
• Rehabilitation Planning:
  Undertake route planning and kitchen assessments to support service users’ independence.
• Duty Functions:
  Attend office‑based duty one day per week and utilise home‑working days for documentation and case write‑ups.
• Team
  
  Collaboration:
  
  Work closely with a multidisciplinary team including Rehab Officers, Community Care Officers, an Assistant Practitioner, and a Team Leader.
• Community Travel:
  Travel across Doncaster to support service users, using a pool car when required.
• Integrated Working:
  Be part of an interprofessional team co‑located with the Adults South Team.

• Registration: HCPC registration is required.
• Experience:
  
  A minimum of 2 years’ experience within a rehabilitation setting is ideal.
• Driving:
  Being a car driver is preferable to support community‑based work.
