POSITION TITLE: In-Home Clinician

General Statement of Responsibilities

The In-home Clinician is responsible for direct services with children, adolescents and their families including but not limited to evaluation/assessment; home & community based services; outreach; case and care management; individual and family therapies; crisis intervention; record keeping; collaboration with other professionals and team members and related community services (including consultation and/or community and/or school placements). He/she is assigned to carry a pager and responds to all calls for assistance either by telephone or in person (as clinically indicated).

She/he provides the majority of treatment services in the home of clients and works a flexible schedule, as based on the needs of assigned cases.

Examples of Responsibilities
  • To provide comprehensive evaluations, treatment plans and treatment of children, adolescents and families. This includes evaluation/assessment; home and community based services; outreach; case and care management; individual, group and family therapies, as well as crisis intervention.
  • To implement treatment plans that may include psychotherapy and clinically required support services including but not limited to translation, transportation (only as per agency policy) advocacy and linkage to other needed services.
  • To provide community based outreach and treatment services as defined by the treatment plan and/or client need, in a variety of settings including but not limited to: client homes, agency sites, schools, emergency rooms, shelters, police stations, etc. The majority of treatment services are provided in the home of the client and will take place primarily in the late afternoon, evenings, overnight and on weekends.
  • To work a flexible schedule as based on the needs of assigned families.
  • To carry a pager and to be available to provide after-hour emergency mobile (in-home, community, emergency room, school-based, other) services including both phone and face to face interventions, as based on agency and/or client needs.
  • Maintain contact and provide follow-up with families requiring this service. This will include daily home visits and home based therapies.
  • Maintain confidential and/or any other required program documents.
  • Maintain a level of productivity as determined by the Program Coordinator and assigned by the program supervisor.
  • To work collaboratively as a member of a team with other professionals and concerned parties involved in cases.
  • To engage families in creative and productive service negotiation and planning.
  • To participate in Child Specific Team case planning and conferences.
  • To facilitate the greatest possible level of family involvement in all facets of program service delivery and decision making.
  • To assist in the organization, marketing, planning and implementation of the program.
  • Actively participate in trainings and staff meetings.
  • To serve on agency and/or community committees.
  • Actively participate in supervision and report directly to the supervisor.
  • To supervise assigned staff and/or student interns.
  • Other such duties as assigned by the Supervisor.
  • Qualifications

    QUALIFICATIONS: Must have the ability to maintain the utmost confidentiality of both agency and client matters. Must understand and be able to display the clinical knowledge and expertise necessary to perform crisis intervention. Must be able to display knowledge and expertise in clinical theory and practice as it relates to the populations served by Wellmore, Inc. Must be capable of working as a part of a team and be able to effectively communicate with both families and professionals (in writing and verbally).
