We are seeking a full‑time Lead Care Manager to play a critical role in our interdisciplinary care model. The position primarily allows remote work, with 2–3 on‑site visits per month in the community alongside a Pair Mate.

• Maintain a caseload of individuals with serious mental illness, substance use disorder, homelessness, or high medical needs using evidence‑based engagement approaches.
• Use relationship‑based strategies to support members with social support navigation.
• Conduct periodic telephonic and SMS outreach to ensure timely follow‑up.
• Collaborate with the care team through systematic case reviews and consults before taking clinical actions.
• Meet monthly encounter metrics to comply with health plan regulations.
• Assist individuals in securing community supports and coordinate physical care appointments.

• 1+ year of general work experience (case management preferred).
• Residency in California with reliable transportation for engagement at clinic and community locations.
• Bilingual (English/Spanish) with strong cultural fluency.
• High degree of empathy, organizational skills, and ability to remain patient when faced with adversity.
• Strong technical skills and comfort with technology innovation, past experience with CRM databases, basic Excel, Word, email, and video conferencing.
• Valid driver's license and auto liability insurance.

• 2+ years of case management experience.
• Experience with motivational interviewing and medical terminology knowledge.
• Detail‑oriented, organized self‑starter who is a rockstar multitasker.
• Preferred lived experience working closely with individuals experiencing complex chronic needs.

• Salary: $22–$25/hour.
