    Program and Position Overview

    The Housing Services Department assists families to secure housing through housing search assistance, move-in financial assistance, eviction prevention, rental subsidies, and home-based case management.

    The Housing Stability Specialist is responsible for providing case management to families during their participation in the rental subsidy program that pulls from service models including Trauma Informed Care, Harm Reduction, Critical Time Intervention and Strength Based Case Management. The Housing Stability Specialist meets at least monthly with each family on their caseload and conducts regular home visits, to support the family to remain stably housed and achieve their goals.

    The case manager works together with the family to establish short- and long-term goals related to housing stability, makes referrals to appropriate resources and assists families to apply for affordable housing opportunities. The case manager is responsible for documenting all services provided, ensuring case notes are entered into the participant’s electronic records in real-time and collecting monthly program compliance documentation from each family.

    This position requires a California Driver License and clean driving record.

    Primary Duties and Responsibilities

    • Provide case management, including home-based case management, to a caseload of approximately 20 families across 6 counties.
    • Case management includes goal setting, housing advocacy, supporting participants and providing community-based referrals for budgeting and financial management, employment, legal services, domestic violence, mental health, and substance abuse.
    • Assess participants for strengths and barriers, including but not limited to employment, behavioral and mental health, support for children’s needs, criminal history, and substance use, and refer participants to appropriate resources.
    • Assist participants to establish and meet housing goals and identify appropriate steps towards achieving goals, including modifying goals as needed, and evaluation of progress.
    • Obtain necessary documentation from participants including income verification, proof of rental payment and documentation of progress towards goals as needed.
    • Complete follow up calls with participants after program completion.
    • Promote values of self-sufficiency and empowerment throughout work with participants using a Strengths Based Model.
    • Reinforce and follow appropriate boundaries with participants.
    • Coordinate and collaborate effectively with internal and external service providers and community partners to assist participants to reach their goals. This may include case conference meetings with other service providers.
    • Maintain participant confidentiality and HIPAA requirements; keep accurate records, files and correspondence. Maintain program standard of documentation of case management services including entries into multiple participant databases.
    • Coordinate with the Real Estate Department at Hamilton Families to maintain positive relationships with landlords and property management companies serving low-income families. This includes supporting families with concerns related to their housing including maintenance issues, lease clarifications, tenant rights, inspections and move out procedures.
    • May engage with the strategic partnerships department to identify resource needs specific to neighborhoods/cities/counties where the families reside.
    • Assist with fun yearly events for families including Holiday celebrations, gift distributions, and Back to School events.
    • Occasional opportunities to assist with various departmental projects and represent the stability department in internal meetings and work groups.
    • Represent and promote Hamilton Families network of housing, rental assistance, and eviction prevention services throughout the community. Represent Hamilton Families in community and collaborative meetings as needed.
