We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.
Responsibilities- The Care Coordinator will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our care plan templates in the electronic health record.
- Abide by the plan of care and orders of the practice.
- Provide prevention and intervention for multiple disease conditions through motivational coaching.
- Develop positive interactions with patients on behalf of our practices.
- Improve revenue by creating billable CCM episodes and increasing visits for management of chronic conditions.
- Develop detailed care plans for both doctors and patients; care plans exist for prevention and intervention purposes.
- Understand health care goals associated with chronic disease management provided by the practice.
- Attend regularly scheduled meetings (e.g., bi-monthly staff meetings, monthly one-on-ones). These meetings are important to define the current scope of work.
- Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
- Current COMPACT license to practice as an RN/LVN/LPN in the state of residence with no disciplinary actions noted.
- Minimum of two (2) years of clinical experience in Med/Surg, Case Management, or home health care.
- Hands-on experience with Electronic Medical Records and familiarity with Windows desktop and applications (Microsoft Office 365, Teams, Excel), in a HIPAA-compliant home environment for CCM duties.
- Ability to exercise initiative, judgment, organization, time-management, problem-solving, and decision-making skills.
- Skilled in using various computer programs.
- High-speed internet and a desktop or laptop computer (Windows or Mac; Chromebooks not allowed).
- Excellent verbal, written, and listening communication skills.
- Quickly recognize condition-related warning signs.
- Organized, thorough documentation skills.
- Self-directed with strong prioritization and time management.
- Clear diction and exemplary phone etiquette.

GEMMS ONE is presented as a cardiology-focused EHR technology with extensive cardiovascular knowledge content and operational tools, including patient portal, e-prescribing, clinical documentation, and practice management with cardiovascular quality measures and a MIPS patient dashboard.
