Licensed Practical Nurse; LPN- Home Care Supervisory Nurse

The Licensed Practical Nurse (LPN) provides skilled nursing services, completes supervisory visits, and supports quality assurance in the field under the direction and supervision of the Registered Nurse Case Manager and agency Administrator. This is a part‑time field position that combines direct client care with oversight of caregiver performance to ensure quality and compliance with Kal Care Health Care policies and state regulations.

• Conduct supervisory visits for personal care and companion clients as required by agency and payer standards.
• Perform skilled nursing visits as needed, including medication administration, wound care, and health assessments.
• Document all visits promptly and accurately in the EMR system.
• Communicate changes in client condition to the supervising RN or Administrator.
• Review and monitor caregiver performance during supervisory visits.
• Educate caregivers on client‑specific needs and proper care techniques.
• Participate in client care conferences and team meetings as requested.
• Maintain compliance with all agency, state, and federal documentation standards.

Part‑time / Flexible scheduling based on client needs and caseload availability.

Metro Atlanta:
Rockdale, Newton, Henry and Dekalb Counties

Weekends not required

Reports directly to the RN Case Manager and/or Administrator.

• Active Licensed Practical Nurse (LPN) license in the state of Georgia (required).
• Minimum 3 year of home care or skilled nursing experience preferred.
• Strong communication and documentation skills.
• Valid Georgia driver’s license and reliable transportation.
