** We are seeking a compassionate
** Adult/Geriatric Nurse Practitioner
** to join our growing home-based care team in the San Fernando Valley & Antelope Valley area. This is a meaningful field role focused on caring for patients where they are most comfortable — at home — during some of the most important moments in their recovery.

You will manage an assigned panel of patients receiving
** acute and transitional care in the home**, including individuals recently discharged from the hospital, emergency department, or skilled nursing facilities, as well as medically complex, homebound patients with lower acuity needs.

* Conduct in-home health assessments and focused physical exams
* Manage acute and post-acute conditions for patients transitioning from hospital, ER, or nursing facilities
* Develop, implement, and document individualized care plans
* Order, interpret, and evaluate diagnostic tests
* Prescribe medications and treatments within scope of practice
* Collaborate closely with physicians, care managers, and interdisciplinary team members
* Educate patients and families to support recovery, safety, and long-term health at home
* Maintain timely, accurate clinical documentation## Service Area This position serves patients throughout the San Fernando Valley & Antelope Valley including:

* Master’s degree in Nursing (MSN)
* Adult/Geriatric Nurse Practitioner certification
* Minimum 1 year of Nurse Practitioner clinical experience (home-based or transitional care experience a plus)
* Active, unrestricted RN and NP license in the applicable state
* Current TB test or willingness to obtain prior to start
