Licensed Vocational Nurse Case Manager (Candidate must reside within 50 miles of Sacramento)
Job DescriptionThe Clinical Case Manager is responsible for coordinating all systems and services required to deliver an organized, multidisciplinary, patient-centered team approach to care for Managed Medi-Cal patients. This role emphasizes cost-effective care while managing and following the course of treatment for patients, in collaboration with physicians, nurses, case managers, and other staff.
Responsibilities- Coordinate all systems and services for an organized, multidisciplinary patient care approach.
- Ensure cost-effective care for Managed Medi-Cal patients.
- Follow and manage the course of treatment for patients.
- Collaborate with physicians, nurses, case managers, and other healthcare staff.
- Experience in healthcare.
- Proficiency in prior authorization processes.
- Strong case management skills.
- Experience in Skilled Nursing Facilities (SNF).
Skills & Qualifications
- Valid California LVN license.
This position offers a remote work setting with a Monday to Friday day shift schedule. The office setup includes dual screens to facilitate efficient work processes.
Job Type & LocationThis is a Contract to Hire position based out of Sacramento, CA.
Pay And BenefitsThe pay range for this position is $38.00 - $43.00/hr.
