Position: Hospice Nurse Case Manager (Multiple Locations)

Role Summary

Our client, a hospice care provider serving patients in a regional Oregon service area, is seeking a dedicated and compassionate Hospice Nurse Case Manager. In this full-time role, the RN will oversee and coordinate comprehensive hospice care, ensuring patients and their families receive high-quality clinical support, education, and compassionate guidance throughout the end-of-life journey.

Key Responsibilities
  • Conduct comprehensive patient assessments to develop and manage individualized hospice care plans.
  • Coordinate and manage patient visits in alignment with physician orders and patient needs.
  • Administer and manage medications, ensuring timely and accurate execution of care plans.
  • Supervise and support LPNs/LVNs and Hospice Aides in accordance with established protocols.
  • Collaborate with an interdisciplinary care team to address patients’ physical, emotional, and spiritual needs.
  • Educate patients, families, and caregivers, providing resources and guidance throughout the hospice process.
  • Maintain accurate clinical documentation in compliance with regulatory and organizational standards.
  • Deliver care with empathy, respect, and professionalism in all patient and family interactions.
  • Participate in an on-call rotation as required.
Required Qualifications
  • Active Registered Nurse (RN) license.
  • Minimum of one (1) year of nursing experience.
  • Willingness and availability to participate in on-call coverage.
  • Reliable transportation and a valid driver’s license for travel within the service area.
  • Strong communication, organizational, and interpersonal skills.
  • Commitment to compassionate, patient-centered end-of-life care.
Preferred Qualifications
  • Associate’s degree in Nursing.
  • Prior experience providing hospice or end-of-life care.
  • Experience coordinating care in home-based or community healthcare settings.
Compensation & Work Model
  • Full-time, field-based hospice role.
