03/02/ Centre View Blvd, Crestview Hills, Kentucky, , United States of AmericaHome Retention Manager is responsible for clinical oversight of the Home Programs (peritoneal dialysis and home hemodialysis). Specific Home Program oversight includes: clinical outcomes, quality improvement, survey readiness, implementation of home remote monitoring capabilities, and creating a Special Place to foster the development and growth of home teammates. Home Retention Manager has leadership responsibility to cover up to two Divisions approximately 60 home programs and work in collaboration with Home Divisional Leads, Regional Home Managers, Clinical Services, Facility Administrators, and Home Nurses to monitor and influence clinical outcomes.
Home Retention Manager are the subject matter expert for home modalities and provide direct clinical support to home leadership, physicians and direct care staff.
ESSENTIAL FUNCTION
Development and implementation of action plans to drive clinical results
- Monitor and conduct trend analysis of clinical outcomes
- Partnering with Home Leadership to implement action plans and monitor results
- Reviewing home remote monitoring data to assist patient management
- Working directly with home nurses to leverage home remote monitoring technology
- Report out on clinical outcomes and action plans to home leadership team
- Foster physician and nurse relationships to implement clinical initiatives
- Evaluating the training of Home Dialysis Nurses to identify opportunities
- Partnering with Home Dialysis Nurse preceptors to train and develop Home
- Nurses
- Attend team meetings, phone conferences, and training as needed
- Travel to and from facilities within the assigned Division as needed
- Know, understand, and follow teammate guidelines, employment policies, and department or company procedures
- Other duties as assigned.

Certificates, licenses, and registrations required or preferred for this position
* RN license in state of employment required or compact licensure
* Valid driver's license in state of employment and positive driving record sufficient to meet insurance requirements of company required
* CPR certification required
TRAVEL AND TIME REQUIREMENTS
Travel: Travel required up to 50% within assigned division
Nights/Weekends: Will be assigned according to the needs of the business
Shift Requirements: Teammates must be willing to work opening, mid and closing shifts according to the needs of the business
