- Serve as a clinical subject matter expert for patients with advanced Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD)
- Lead complex Goals of Care discussions and advance care planning, ensuring alignment with patient values
- Educate patients on Advanced Directives and facilitate completion to prevent unwanted interventions
- Assess symptom burden and implement nursing interventions to improve Quality of Life
- Coordinate care transitions and management for high-complexity patients
- Minimum of 2+ years of experience as an RN in Palliative medicine
- Minimum of five (5) years of experience as an RN
- Current RN License is required; BSN preferred
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Proficiency in EMR systems and Microsoft Office
- Experience as an Integrated Kidney Care RN Case Manager
- 2+ years of Case Management or Chronic Care Management experience
- Minimum of two (2) years of experience in renal nursing preferred
- Compact License is highly preferred
