• Coordinate with physicians, rehab services, and other care providers
• Educate client on diagnosis and treatment, ensuring understanding
• Provide skilled nursing services including wound care, medication management, and managing chronic conditions
• Remain highly organized in providing and documenting care

• Current, unrestricted California LVN license
• Graduation from accredited practical or vocational nursing program
• 1 year of experience as a Licensed Vocational Nurse
• Home Health experience preferred
• Proof of Negative TB result
• Physical Screening documentation within last year
• Current BLS certification
• Valid California drivers license and car insurance
• Cell phone with internet access

• Hourly Rate: $35-$50, depending on experience, certifications, reference reviews, car access, and more
