Supportive Care Case Manager RN - REMOTE

• Support complex patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) by navigating the healthcare system
• Coordinate care and facilitate communication among the healthcare team, including Operations Manager, Clinical Services Manager, and Case Managers
• Educate patients on treatment options and assist in developing care plans aligned with their goals and preferences
• Build empathetic relationships with patients and families to support their needs and facilitate advance care planning
• Manage care transitions and ensure effective discharge planning for patients

• Minimum of 2+ years of experience as an RN in hospice or palliative medicine
• At least five (5) years of experience as a registered nurse (RN)
• Current RN license is required; BSN preferred
• Current CPR or BLS certification
• Intermediate computer skills and proficiency in MS Word, Excel, and Outlook

• Experience as a DaVita Integrated Kidney Care RN Case Manager
• Residing in the Mountain or Pacific Time zones
• 2+ years of experience in case management or chronic care management
• Minimum of two (2) years of experience in renal nursing
• Spanish-speaking proficiency
