Summary

The position is eligible for Education Debt Reduction Program (EDRP) - student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200
-000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

• The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Geriatrics and Extended Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
• The NP provides advanced clinical practice - consultation - and management
• The NP practices to the full extent of their education - training - and certification
• Core functions are outlined in the clinical privileges that correspond with this position
• The major duties and responsibilities include - but are not limited to:
  Assesses and supports the caregiver's capability to continue to provide care
• Helps develop and approve patient treatment plans based on a comprehensive interdisciplinary assessment of the patient and the caregiver
• Functions as the Primary Care provider for assigned HBPC Veterans and provides clinical oversight of assigned staff
• Implements the Veteran's treatment plan through home visits
• Provides education - counseling and emotional support for patients and families receiving care
• Instructs medical/nursing students - residents - and other learners that rotate through GEC Service
• Coordinates Veteran care with other members of health care team
• Obtains and evaluates medical histories - performing physical examinations - therapeutic and diagnostic procedures - and ordering laboratory studies - radiographs - electrocardiograms - and other special examinations
• All other duties as assigned
