Overview

We are seeking a dynamic, passionate Registered Nurse (RN) to be part of a virtual nursing team in home and community care. The role provides clinical leadership and support to nurses across the province and virtual care delivery to clients in a client-centered approach. You will work with the Professional Practice leadership team to support excellence in practice through knowledge translation methods, using evidence-informed nursing knowledge, skill, and judgment.

The goal is to support optimal RN and RPN scope of practice to provide holistic nursing care addressing the physical, safety, emotional, and spiritual needs of clients and their families in their homes and communities.

Responsibilities
  • Provide clinical leadership and support to nurses across the province and through virtual care delivery.
  • Deliver nursing assessment, management and virtual care to clients and educational support to caregivers through digital technology.
  • Partner with Advanced Practice Leaders to develop evidence-based clinical practices for home and community clients.
  • Liaise with regional Clinical Practice Coaches and Nurse Managers to support knowledge translation of clinical best practices to the front-line.
  • Develop nursing care plans and adjust treatment codes as required.
  • Monitor practice and staff needs through analysis of reports; contribute to organizational innovation.
  • Assist with follow-up with funders and/or physicians to ensure referrals and service updates are complete.
  • Demonstrate commitment to continuous learning and professional development through formal and informal learning mechanisms.
  • Support interdisciplinary care and the nursing professional practice model in partnership with the Advanced Practice leadership team.
  • Assist the Advanced Practice leadership team with reviews and development of policies and procedures.
Qualifications
  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Master of Nursing preferred;
    Bachelor of Science in Nursing required.
  • Current expertise/certifications in Older Adult/Geriatrics, Chronic Disease Management, General medicine/surgical, Infusion Therapy (preferred).
  • Recent pediatric experience (preferred).
  • Canadian Nurses Association or North American certifications (preferred).
  • Minimum 3–5 years of recent community nursing experience with knowledge of the organization’s programs and services.
  • Comfortable using technology, including telephone systems, software programs, and computer programs.
  • Private home work environment with high-speed internet.
  • Flexible, organized, capable of taking initiative and leading with adaptability to changing demands.
  • Excellent interpersonal, communication, problem-solving, and critical-thinking skills.
  • Sound decision-making to plan, organize, and implement proactively and responsively.
  • Ability to work independently and as part of a collaborative team.
  • Appreciation for Quality Improvement processes and willingness to attain SE Health certifications.
  • Availability to work days, evenings, nights, and weekends.
  • Willingness to be cross-trained to other services within the Center of Excellence in Virtual Care.
  • Vehicle and valid driver’s license; ability to travel and attend in-person meetings as required.
  • Updated immunization records, clear criminal background check, and Vulnerable Sector Check required upon hire.

Total compensation for this position is $69,000–$87,000, determined based on the successful candidate’s relevant experience, skills, and competencies, taking into consideration internal equity.
