Registered Nurse (RN) – Acute Care, Home-Based

Lake County, IL. We are partnered with a rapidly growing Hospital at Home program delivering acute-level care directly to patients in their homes. This innovative model allows experienced RNs to practice with a high level of autonomy while maintaining strong clinical support, predictable scheduling, and meaningful patient interaction.

• Full-Time: 3 x 12-hour shifts
• Shift: 7:00 AM – 7:00 PM

As a Field RN, you will:

• Conduct 6–7 patient visits per shift
• Spend up to 50 minutes per patient
• Deliver acute-level nursing care in the home setting
• Collaborate daily with hospitalists via telehealth
• Begin and end shifts at a local command center

This role blends hands-on patient care with structured support, documentation time, and team collaboration.

• Morning huddle and interdisciplinary team meeting at the command center
• Field visits between 8:00 AM – 12:00 PM
• Midday documentation and charting
• Afternoon patient visits
• Return to command center by end of shift

• Company vehicle provided
• Fully stocked supply room (restocked from hospital)
• Medications coordinated through inpatient pharmacy
• Laptop and mobile documentation tools provided
• Real-time safety tracking via secure location-based app

Required

• Active RN license
• Hospital experience required

Preferred

• Critical Care, ICU, or step-down background
• Acute inpatient experience
• BSN strongly preferred (or actively enrolled in a BSN program)
