Taxable Pay: $1,368.00

The facility prefers no RTO (Requested Time Off) for this contract.

🩺 Role Highlights
Hospice Case Management:
Act as the primary clinical lead for a rural hospice caseload, coordinating care between families, physicians, and interdisciplinary teams.

Compassionate Home Care:
Deliver end-of-life care in the comfort of patients' homes, managing complex symptoms and providing emotional support.

Rural Navigation:
Navigate the beautiful but varied rural landscape of Central Vermont to reach patients in need.

EMR Efficiency:
Utilize McKesson/Netsmart for streamlined documentation and care tracking.

📋 Mandatory Requirements
Licensure:
Active Vermont (VT) or Compact/Multi-State RN License.

Core

Experience:

Verifiable experience in Hospice Case Management and Home-Based Hospice Care is required.

Rural Comfort:
Prior experience with rural travel and geographic navigation is preferred.

Tech

Skills:

Experience with McKesson/Netsmart EMR is highly preferred.

Availability:
Must be able to commit to the weekend-heavy schedule without time-off requests.
