Are you a compassionate Registered Nurse looking to combine your clinical skills with a flexible, hybrid schedule? Join our mission-driven team as an RN Care Manager , where you’ll support family caregivers and adults with complex medical needs through a blend of in-home visits and remote coaching. This full-time opportunity offers a competitive salary ($90,000–$105,000) and a $5,000 sign-on bonus —no night shifts, weekends, or on-call required.

• Conduct in-person home visits throughout your assigned territory to assess safety, observe health changes, and provide hands-on clinical coaching.

• Follow person-centered care plans and deliver ongoing virtual support through our digital care management platform.

• Build strong, trusting relationships with caregivers to promote sustainable caregiving practices and reduce burnout.

• Perform medication reviews, offer best practice recommendations, and connect families to appropriate community resources.

• Accurately document all visits and interactions, maintaining compliance with regulatory standards and internal protocols.

• Participate in team projects and take on additional responsibilities as needed.

• Active Registered Nurse (RN) license in the state of Massachusetts.

• Minimum 2 years of recent experience in direct care, preferably with elders, adults with disabilities, or individuals with complex care needs.

• Valid driver’s license, reliable vehicle, and current auto insurance —local travel is required.

• Background in case management, care coordination, or clinical assessments strongly preferred.

• Comfort with technology, including MS Office and electronic case management systems.

• Bilingual candidates are highly encouraged to apply and may be eligible for enhanced compensation.

• Boston

• East Boston

• Watertown

• Chelsea

• Revere

• Roslindale

• Hyde Park

This hybrid role allows you to work from home while scheduling visits at your own pace within your designated territory.
