Overview
Integrated Care Clinician (Nurse)
The post holder will undertake remote telephone assessments of patients in an efficient way. They will advise the most appropriate intervention to meet the needs of the patient using best clinical practice, adhering to Trust Policy, protocols and guidelines. They will work in an autonomous capacity, assessing and advising callers within agreed guidelines.

- Advise the most appropriate intervention to meet patient needs using best clinical practice and in line with Trust Policy, protocols and guidelines.
- Work in an autonomous capacity, assessing and advising callers within agreed guidelines.
- Draw on clinical assessment skills to support a broad spectrum of patients with minor ailments, injuries, complex or urgent conditions in a remote clinical decision-making environment.
- Collaborate with urgent care GPs, pharmacists and other colleagues as part of a multidisciplinary team.
111 Wales/IC is the service that gives the people of Wales access to urgent out of hours care, 24-hour health information with signposting and access to urgent dental services. The service aims to deliver a clinically effective, patient-centred experience across Wales, online and over the phone. We work with a wide range of colleagues, including Call Handlers, Nurses, Paramedics and data analysts, as part of a multidisciplinary team alongside GPs, Pharmacists and Mental Health Practitioners.
