Overview

Integrated Care Clinician (Nurse) The post holder will undertake remote telephone assessments of patients in an efficient way. They will advise the most appropriate intervention to meet the needs of the patient using best clinical practice, adhering to Trust Policy, protocols and guidelines. Will work in an autonomous capacity, assessing and advising callers within agreed guidelines. This is a dynamic role in which you’ll draw upon your clinical assessment skills to help and advise a broad spectrum of patients, from minor ailments and injuries to complex and urgent conditions as well as to hone your skills in a remote clinical decision-making environment.

Our highly trained teams work closely with urgent care GPs and pharmacists in looking after Wales Urgent care needs. Working for 111 Wales/IC will mean you make a real difference to people’s lives and health as we engage with thousands of patients each week.

Responsibilities
  • Undertake remote telephone assessments of patients in an efficient way.
  • Advise the most appropriate intervention to meet patient needs, using best clinical practice and adhering to Trust Policy, protocols and guidelines.
  • Work autonomously, assessing and advising callers within agreed guidelines.
  • Draw upon clinical assessment skills to help and advise a broad spectrum of patients, from minor ailments to complex and urgent conditions, in a remote clinical decision-making environment.
  • Collaborate with urgent care GPs, pharmacists, and a multidisciplinary team as part of the wider 111 Wales/IC service.
  • Engage with thousands of patients each week and contribute to a clinically effective, patient-centred service.
Qualifications / Experience
  • Experience as a nurse involved in remote or triage-based care is implied by the role title, with emphasis on clinical assessment and decision-making.
  • Ability to perform remote clinical decision-making and provide appropriate advice in line with guidelines.
  • The ability to speak Welsh is desirable;
    Welsh and/or English speakers are equally welcome to apply.
