Nurse Practitioner - Home Based Primary Care

Advanced Practice Nurse (APN) III Certified Nurse Practitioner

Primary Care Service, Mental Health Primary Care, Medical and Surgical assignments in both inpatient and outpatient clinical settings.

• Independent panel management of acute and chronic health conditions with emphasis on health promotion and disease prevention.
• Develop and implement individualized plans to achieve optimal population health and disease management.
• Order and interpret labs, tests, and diagnostic studies.
• Prescribe pharmacologic and non-pharmacologic interventions.
• Initiate referrals and consults to specialty services.
• Provide patient education and family counseling.
• Assess and adjust care plans to improve outcomes.
• Demonstrate leadership and collaborate with Primary Care Team and interdisciplinary team members.

• U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
• Minimum 2–3 years of NP experience in a Primary Care Clinic in an acute care medical center, Department of Veterans Affairs RN or NP experience.
• 1 full and unrestricted state or territorial license for NP practice.
• Credentialing appropriate NP national certification.
• English language proficiency; basic written and spoken English.
• Background/security investigation.
• Pre-employment physical examination required.
• Participation in the seasonal influenza vaccination program required.

Monday – Friday, 8:00 am – 4:30 pm.
