Were seeking a passionate and driven Physician Assistant/Nurse Practitioner to join our Geriatrics team in Grand Rapids, Michigan. This is a dual role supporting both the Geriatrics Home-Based Primary Care (HBPC) program and the Geriatrics Primary Care Clinic. This is an exciting opportunity to make a meaningful impact on the health of our community while working in a supportive, mission-driven environment.
Opportunity Details
- Position type: Full-Time (1.0 FTE)
- Location: Grand Rapids, MI
- Specialty: Geriatrics Home-Based Primary Care (HBPC) program and the Geriatrics Primary Care Clinic
- Schedule: Monday to Friday, 8am to 5pm
- Call requirements: Shared between the providers on team
- Role requirements: The position requires strong clinical judgement, exceptional communication skills, and the flexibility to transition between home-based care and clinic-based care to meet the needs of geriatric patients with complex medical, functional, and social needs.

Requirements
- Provider experience preferred, open to new graduates with strong nursing experience
- APPs: Board certified in specialty prior to employment
- Active Michigan medical license (or ability to obtain)
- DEA license
- ACLS/BLS certification (must be maintained)
Duties & Responsibilities
A typical day may include:
- Providing direct patient care
- Collaborating with multidisciplinary teams
- Participating in teaching and mentoring
- Documenting in Epic EMR
- Engaging in quality improvement initiatives
- Supporting community health efforts
