Remote Monitoring II
- 6 years or more RN experience with ASN Degree OR BSN
Remote Monitoring III
- 12 years or more RN experience with ASN/ADN Degree OR 6 years or more RN experience with BSN OR Masters/Doctorate of Nursing
The Registered Nurse provides direct nursing care in accordance with established policies, procedures and protocols of the organization. The nurse maintains the knowledge and skills necessary to provide care appropriate to the age of the patients served. The RN Remote Patient Monitoring champions the home-based remote patient monitoring program by facilitating implementation, educating patients and providers and monitoring readings and outcomes.
The RN facilitates the coordination of patient care within the interdisciplinary care team and provides patient education. The RN communicates/collaborates with the patient, interdisciplinary team, and providers in providing care.
- Implements, evaluates, and maintains the remote patient monitoring program, including patient and clinician education and skills validation, policy and procedure review development including reviewing and updating, proper care and maintenance of equipment and serving as a resource to patients and end users.
- Collaborates with remote patient monitoring vendor on a regular basis.
- Evaluates patient status using remote monitoring data, identifies early signs of clinical deterioration, and responds appropriately by providing care aligned with the patient’s diagnoses, symptoms, and provider orders, including notifying the responsible provider when indicated.
- Communicates and collaborates with the patient, interdisciplinary team, and providers in providing care. Ensure that intervention or referrals are made promptly.
- Promotes patient/caregiver education using various verbal and written communication techniques that consider the patient/caregiver’s cultural, ethnic, and personal needs or preferences.

- Basic Life Support (BLS) | American Heart Association (AHA) | Within 90 days of Start Date

- Temporary RN license accepted prior to start.
- Must hold an unencumbered nursing license prior to start.
