Role Description

This is a full-time hybrid role for a Registered Nurse (RN) - Hospice based in Ft. Worth, TX, with some flexibility to work from home. The RN will provide palliative and hospice care to patients, assess individual health needs, administer medications, and offer emotional support to patients and their families. The role involves coordinating with physicians, caregivers, and other members of the care team to ensure a high standard of patient care while focusing on comfort and quality of life.

The nurse will also educate patients and families about care plans and end-of-life considerations while documenting patient progress accurately.

Qualifications
  • Skilled in Nursing and Direct Patient Care
  • Expertise in Palliative Care and Home Care services
  • Knowledge and experience in Medical and Hospice-related care
  • Compassionate communication and interpersonal skills
  • Current Registered Nurse (RN) license in the state of Texas
  • Proficiency in medical documentation and care planning
  • Ability to work both independently and as part of a collaborative care team
  • Previous experience in hospice or palliative care is a strong advantage
