Position SummaryWe are seeking an experienced and compassionate Hospice Registered Nurse (RN) to provide clinical visits, case management, and coordination of care for patients in the Torrance and Carson areas. You will work closely with the interdisciplinary hospice team to ensure patients and families receive the highest level of support and comfort.
Responsibilities- Provide skilled nursing assessments, symptom management, and patient education
- Develop and update individualized care plans
- Coordinate with physicians, social workers, and spiritual care providers
- Administer medications and treatments as prescribed
- Support and educate family members and caregivers
- Maintain timely and accurate clinical documentation
- Current California RN license (required)
- Minimum 1 year of nursing experience (Hospice or Home Health preferred)
- Valid driver’s license, auto insurance, and reliable transportation
- Compassionate, dependable, and comfortable working independently
